غربالگری سرطان دهانه رحم: بررسی کارآیی مداخله آموزشی مبتنی بر نظریه انگیزش محافظت بسط داده شده

زمینه و اهداف: از روشهای مؤثر در غربالگری سرطان دهانه رحم، آزمایش پاپ اسمیر است. متاسفانه بسیاری از زنان از انجام آن سر باز می زنند. این پژوهش، با هدف بررسی مداخله آموزشی مبتنی بر نظریه انگیزش محافظت بسط یافته بر رفتار غربالگری سرطان دهانه رحم اجرا شد.
مواد و روش ها: پژوهشی نیمه تجربی ، بین 200 نفر از زنان مراجعه ­کننده به مراکز بهداشتی شهر کرج اجرا شد (هرگروه 100نفر، تخصیص افراد در هر گروه بصورت تصادفی). ابزار گردآوری داده ها پرسشنامه استاندارد انگیزش محافظت شامل متغیرهای جمعیت شناختی، سازه های نظریه انگیزش محافظت و پرسشنامه محقق ساخته حمایت اجتماعی عاطفی بود. گروه­ ها قبل، بعد و 6 ماه پس از مداخله آن را تکمیل کردند. 8 جلسه آموزش با روشهای سخنرانی، بحث گروهی و ایفای نقش مبتنی بر الگو ، برای گروه آزمون اجرا شد. تحلیل داده ها با نرم­افزار24SPSS  و آزمون تحلیل واریانس اندازه گیری مکرر و کای اسکوئر انجام شد.
یافته ها: در مراحل قبل و بعد از مداخله میانگین نمرات سازه های حساسیت درک شده از28/3±07/8 به99/1±83/11، شدت درک شده از 46/3±64/12به 85/2±19/15، کارآیی پاسخ از37/5±66/12به 6/3±06/16، خودکارآمدی ادراک شده از 38/5±94/25 به 68/4±68/29، حمایت اجتماعی عاطفی از 16/7±01/27 به  76/5±46/31 و قصد از 14/2±01/11 به 65/1±42/13 افزایش داشت که از نظر آماری معناداربود (05/0>p). میانگین نمره ترس از29/2±37/10 به 76/2±54/5 و هزینه پاسخ از13/2±62/6 به 69/1±97/3 کاهش یافت که از نظر آماری معنادار بود (05/0>p). در گروه کنترل اختلاف معناداری دیده نشد. انجام پاپ اسمیر بعد از مداخله در پس آزمون و پیگیری در گروه آزمون افزایش آماری معنی داری داشت (05/0>p).
نتیجه ­گیری: براساس نتایج، می­توان از نظریه انگیزش محافظت و سازه حمایت اجتماعی عاطفی به عنوان چارچوبی در طراحی برنامههای آموزشی برای ارتقای رفتار غربالگری سرطان دهانه رحم استفاده کرد.
